Change of Address

A licensee must notify the Plumbers Licensing Board of a change in the licensee’s residential or business address within 14 days of the change.

 Note: Late notification of changes may result in a penalty.

How to notify the PLB

Notification of changes of address can be done by the following means:

• Complete change of Address form on the back of renewal notice and send to the address below; or,
• Download and complete Change of Address form and send to the address below.
