When to Lodge a Multi-entry Certificate

 All minor plumbing work carried out must be certified on a multi-entry certificate. A multi-entry certificate will record up to 25 individual jobs and will need to be lodged with the Plumbers Licensing Board within 5 working days after the end of each calendar month. For example, all minor plumbing work carried out in July 2009 will be need to be certified on a multi-entry certificate that must be received by the PLB no later than 7 August 2009. 

 A multi-entry certificate will only be able to be lodged by the licensed plumbing contractor to which it is registered and it must be filled out in its entirety. Partially filled or unreadable certificates will not be accepted.

 A licensed plumbing contractor who lodges a multi-entry certificate must keep the certificate at his or her principal place of business for 6 years.

How to Lodge a Multi-entry Certificate

 If a licensed plumbing contractor has carried out minor plumbing work a multi-entry certificate detailing the minor work must be lodged with the Plumbers Licensing Board within 5 working days of the end of each calendar month.

 Multi-entry certificates can be lodged:

  1. in person at any Commerce office
  2. by posting to:
    Plumbers Licensing Board
    Locked Bag 12
    WEST PERTH WA 6872; or
  3. by faxing to 1300 449 185, provided the original is submitted to the Board within one week of receipt of the facsimile.
